Summary
The Team Nurse is a member of the a cross-disciplinary team that includes health care providers, medical assistants, nurses, behavioral health providers and other support staff. They support the mission and values of Maple City Health Care Center (MCHCC) by collaborating with team members to facilitate appropriate care of patients and working with patients to engage motivation for change.
Duties and Responsibilities
- Collaborates with the cross-disciplinary care team to address the wholistic needs of patients
- Engages patients to address patient needs, utilizing Motivational Interviewing and Trauma Informed Care approach
- Receives and responds to patient phone calls/MyChart patient advice requests
- Communicates with and educates patients via phone or MyChart
- Completes documentation in a timely and clear manner
- Refills patient medications
- Completes patient forms and letters
- Coordinates with outside entities for patient care
- Maintains skills competency by participating in scheduled trainings
- Other duties that support our shared work, as assigned
- Demonstrates effective interpersonal skills
Job Requirements
- Nursing degree; BSN preferred
- State of Indiana licensure, Registered Nurse preferred
- Basic Life Support Certification
- Trauma Informed Care approach preferred
- Motivational Interviewing training preferred
- Bilingual (Spanish/English)

Accountability
The Team Nurse is supervised by the site Nurse Lead. Details of review processes are outlined in the employment agreement and in the Employee Handbook.
Salary Range and Benefits
Salary is determined according to the Employee Salary and Wage Protocol, based on a range of factors including education, experience, responsibility, and language proficiency. They will receive comprehensive employee benefits package, based on FTE.
